If you are HIV positive and are in need of services, call Welcome House, Inc. at (214)887-0696 to schedule an intake with the Client Advocate. Be prepared to provide the following information:

        ·       A letter of diagnosis.
·        Photo Identification (any of these, for example: photo ID, TX drivers license, etc.)
·        Social Security Card.
·        Proof of income for the current year (any of these, for example: check stub, award letter from SSDI, copy of paycheck, etc.)
·        Proof of residency (any of these, for example: copy of lease agreement, utility bill with name and address, correct drivers license, bank deposit slip, etc).

Intakes are typically scheduled within three to five business days, Monday through Friday, from 9 AM until 5 PM. You can come to our Welcome House, Inc. office or we can come to your home, depending on the circumstances. Welcome House, Inc.  also serves the family member, partner, or caregiver of someone living with HIV/AIDS.
